How Do I Know If I Need HRT? Signs It May Be Time to Talk About Hormone Therapy

If you’ve found yourself Googling “HRT near me” or wondering whether your symptoms could be related to perimenopause or menopause, you’re definitely not alone.
Many women come to me saying some version of:
“My labs were normal, but I don’t feel like myself.”
“I’m exhausted all the time.”
“My periods are suddenly different.”
“I’m gaining weight even though I haven’t changed anything.”
“My anxiety is worse.”
“I have no interest in sex anymore.”
“I’m waking up at 3 a.m. every night.”
And one of the biggest questions they have is: Do I actually need hormone replacement therapy?
The answer isn’t based on one symptom or one lab result. Hormone therapy is individualized, and deciding whether it’s appropriate starts with looking at the bigger picture.
What is HRT?
Hormone replacement therapy, or HRT, is treatment used to replace or supplement hormones that decline during perimenopause and menopause.
You may also hear the term BHRT, or bioidentical hormone replacement therapy. “Bioidentical” describes hormones that are chemically identical to hormones produced by the human body. FDA-approved hormone therapy options can be bioidentical, including certain forms of estradiol and micronized progesterone.
HRT may include estrogen, progesterone, or other treatment depending on your symptoms, health history, stage of life, and individual needs.
Signs It May Be Time to Have a Conversation About HRT
Perimenopause doesn’t look the same for every woman. And it isn’t limited to hot flashes.
Women may experience:
• Changes in their menstrual cycle
• Hot flashes or night sweats
• Difficulty falling or staying asleep
• Brain fog or difficulty concentrating
• Mood changes, anxiety, or irritability
• Fatigue or decreased energy
• Vaginal dryness
• Pain or discomfort with sex
• Decreased libido
• Changes in body composition or weight
• Joint or muscle discomfort
• Feeling like you simply “aren’t yourself anymore”
Having these symptoms doesn't automatically mean that you need HRT. But if they are new, persistent, or interfering with your quality of life, they are worth discussing with a healthcare provider who understands perimenopause and menopause.
But What If My Hormone Labs Are “Normal”?
This is one of the biggest sources of frustration I hear from women.
A woman can have symptoms of perimenopause even when a single hormone panel appears to fall within a laboratory's reference range.
Hormone levels can fluctuate considerably during the menopausal transition. For many women over 45 with typical symptoms, major guidelines recommend that perimenopause and menopause be assessed primarily using age, menstrual history, symptoms, and clinical history rather than relying on a single hormone test alone.
That doesn't mean labs are useless.
Depending on your symptoms and medical history, testing can still be helpful to evaluate other possible contributors, such as thyroid dysfunction, anemia, metabolic concerns, nutrient deficiencies, or other conditions that can overlap with symptoms commonly attributed to hormones.
The goal should be to look at you as a whole person, not just whether one number has an H or L next to it.
Is HRT Safe?
There isn't one answer that applies to every woman.
For many healthy women who are younger than 60 or within approximately 10 years of menopause onset and don't have contraindications, the benefit-risk profile of menopausal hormone therapy can be favorable, particularly for bothersome vasomotor symptoms.
But HRT isn't appropriate for everyone.
Your personal and family medical history, age, symptoms, medications, cardiovascular risk, breast health, history of blood clots, abnormal bleeding and other factors all need to be considered before deciding whether hormone therapy is appropriate.
That's why I don't believe hormone care should start with:
“Your estrogen is low, so here's a prescription.”
It should start with a conversation.
What Happens at a Hormone Assessment?
